System Recovery & OS Troubleshooting
In-depth case study on diagnosing, repairing, and recovering mission-critical enterprise endpoints from EFI boot loop, BCD corruption, and file system errors using Hiren's BootCD PE & MediCat USB.
Project Objective
This initiative aimed to establish a standardized, non-destructive disaster recovery playbook for workstations suffering from critical boot failures (0xc00000e, BSOD boot loops, EFI partition corruption) following an improper shutdown during a system upgrade. The goal was to achieve a 100% user data retention rate and restore full OS operational integrity without performing a disruptive OS reinstall.
Environment & Tooling Setup
The diagnostic environment was built around a multi-boot Ventoy flash drive loaded with isolated repair environments to safely service encrypted NVMe drives.
| Component | Specification / Tool | Role in Incident |
|---|---|---|
| Target Hardware | Dell Precision 5820 Workstation (1TB NVMe, UEFI, GPT) | Corrupted Endpoint |
| OS Environment | Windows 11 Enterprise (23H2, BitLocker Encrypted) | Unbootable Target |
| Boot Suite | MediCat USB v21 / Ventoy Boot Manager | Hardware S.M.A.R.T. & Booting |
| PE Environment | Hiren's BootCD PE x64 (WinPE v1.0.2) | CLI Servicing & File Backup |
| Diagnostic Tools | CrystalDiskInfo, TestDisk, Macrium Reflect PE | Health Analysis & Storage Imaging |
Incident Diagnostics & Root Cause
Initial inspection of the target machine indicated an immediate boot halt displaying Windows Stop Code INACCESSIBLE_BOOT_DEVICE followed by error status 0xc00000e (The Boot Configuration Data for your PC is missing or contains errors).

Step-by-Step Remediation Workflow
Step 1: Hardware & Storage S.M.A.R.T. Health Check
Booted into MediCat USB and launched CrystalDiskInfo PE. The NVMe SSD showed a healthy status (98% remaining health, 0 bad sectors), ruling out physical controller or flash degradation.
Step 2: BitLocker Volume Unlocking & File System Repair
Booted into Hiren's BootCD PE environment, opened an elevated Command Prompt, and unlocked the volume via the enterprise recovery key:
Step 3: EFI System Partition Re-formatting & BCD Rebuild
Using diskpart, the hidden EFI System Partition (ESP) was identified, assigned a temporary drive letter, formatted back to FAT32, and populated with a clean boot environment directly from the Windows OS volume:
Step 4: Offline System File Repair via DISM & SFC
To resolve residual OS corruptions, an offline System File Checker scan and DISM repair were run pointing directly to the WinPE recovery source:
Validation, Results & Key Metrics
Following the command execution, the temporary drive mappings were cleared, BitLocker protection was re-enabled, and the system was rebooted into native UEFI mode. The endpoint initiated a smooth cold boot directly into Windows 11 within 12 seconds.
| Verification Test | Expected Outcome | Result |
|---|---|---|
| UEFI Boot Sequence | Direct boot into Windows Login Screen | Passed |
| System File Integrity | Zero integrity violations reported by SFC | Passed |
| Data Integrity Verification | User profiles, documents & certificates intact | Passed (100%) |
| BitLocker Status | Protectors active with valid TPM linkage | Active |
Key Learnings & Hardening Plan
This incident highlighted the importance of having pre-built, multi-boot USB media provisioned with BitLocker CLI management tools in field service kits. Future resilience steps include:
- Deploying uninterrupted power supplies (UPS) across all high-value developer workstations.
- Automating weekly VSS shadow copy creation and Macrium Reflect endpoint backups to offsite network storage.
- Standardizing Tier-2 IT support toolkits with pre-loaded MediCat/Hiren's bootable drives containing active vendor drivers.
